** IMPORTANT PLEASE READ AS THIS WILL EFFECT YOUR VERIFICATION RESPONSE: **

If you require a meeting verification or proof of attendance, please complete the below form daily and ideally straight after the end of the meeting each day. You must include the correct daily meeting attendance code the chairperson says, displays on screen and puts in the Zoom chat as this is required for verification (You must supply the correct attendance code and this is not the NKC Zoom Meeting ID. If the attendance code is incorrect or missing, we cannot provide a verification). Please note: do not send on mass for verification of retrospective dates or for a retrospective date as this is outside of our remit and will not be provided. Please only send one verification request per meeting, otherwise this will delay your response. You must also attend more than two thirds (40 minutes or more) of the main one hour meeting to obtain a meeting verification or proof of attendance. We also recommend you have a unique Zoom Display Username and use the same name in the form to ensure a prompt response to your request. We only provide meeting verification or proof of attendance electronically and in our email format which is an accepted format. If you do not complete and submit the below form on the same day of the meeting this may delay your request or you may not get a meeting verification or proof of attendance.
